House Call Program

Salem Laboratory is pleased to offer phlebotomy house call services for homebound patients in our direct service area. The house call program is available to providers with admitting privileges to Salem Hospital who refer the majority of their patients and/or specimens to the Salem Laboratory.

Eligibility

  • A beneficiary is determined to be homebound if he/she has a health condition due to an illness or injury which restricts his/her ability to leave his/her place of residence or if leaving his/her home is medically contraindicated.
  • Ambulatory patients without transportation do not meet the criteria of homebound as defined above and therefore do not qualify for this service.
  • Services are generally provided for management of medications such as coumadin which require periodic monitoring.
  • Routine blood work for yearly appointments, follow up blood work for elevated blood work should be scheduled and completed at the Salem Hospital Laboratory or one of the draw stations throughout the community. Exceptions will be reviewed by the manager before the draw can be scheduled.

  • Beverly, Boxford, Danvers, Essex, Gloucester, Hamilton, Ipswich, Lynn, Lynnfield, Manchester, Marblehead, Nahant, Peabody, Revere, Rowley, Salem, Saugus, Swampscott, Topsfield, Wenham.
  • Some towns may have selected days for house calls.

Scheduling

  • Requests must be made at least two business days in advance.
  • House call requests are accepted by the Home Draw Department during the hours of 7-2:30 pm, Monday thru Friday.
